The nasal shape can make a profound impact on your appearance. The male nose is often larger than most women’s noses. It’s typically wider and projects further than most female noses. Men also tend to have a straight nasal bridge or one with a dorsal hump, and a nasal tip that is either straight or downturned.
Dr. Solomon is an artist when transforming a masculine nose into a feminine nasal appearance. He masterfully feminizes the nose by incorporating a gentle slope, narrowing the nasal bridge and lifting the nasal tip. The position of the brows can significantly feminize the face for patients transitioning from male to female. Male brows typically sit directly on the brow bone if not slightly below. Brow lift surgery is performed under general anesthesia and can be carried out through an open incision, endoscopically or laterally in a temporal brow lift. A brow lift can often be performed in conjunction with a brow bone reduction as both procedures utilize the same incision along the hairline.
A common masculine facial characteristic is the brow bone that can often protrude from the male forehead. Some males may have more prominent brow bones than others. Dr. Solomon skillfully performs a brown bone reduction under general anesthesia to reduce the projection of this skeletal structure. Brow bone reduction is often combined with a brow lift as the combination of the two procedures can significantly transform and feminize the male face.
Forehead height and its overall size can certainly feminize the male face. A smaller forehead can appear more dainty and feminine compared to a large forehead and high hairline that is viewed as more masculine. Dr. Solomon commonly performs hairline lowering on his patients transitioning from male to female as it can drastically impact ones’ overall appearance.
Hairline lowering, brow lift and brow reduction all require the same incision along the hairline which can make this combination of procedures very appealing to patients. By reducing the height of the forehead, minimizing the appearance of the brow bone and raising the eyebrow position, patients can achieve a transformative and feminine outcome.
The Adam’s apple is a landmark masculine trait that many of our transitioning patients inquire about.
The Adam’s apple is removed through a small incision in the neck, anterior to the tracheal cartilage, which is shaved down Dr. Solomon to create a smooth aesthetic appearance. Laryngeal shave can be performed alone or in conjunction with other facial feminization procedures.
Beautiful full lips are often the desire of females in general as well as males transitioning to female. Injectable filler is a common treatment for lip enhancement, however a modified upper lip lift can reposition the upper lip permanently to feminize the face and give greater volume to the upper lip.
A long white lip (the distance between the base of the nose and the vermillion border of the upper lip) is more masculine in appearance. By shortening the white lip with a lip lift, you can create more dental show and a beautiful aesthetic appearance. A modified upper lip lift can also be combined with dermal filler for added volume.
Chin liposuction, also known as neck liposuction, is a surgical procedure aimed at removing excess fatty tissue to create a more defined chin and jawline. Neck liposuction permanently removes excess fat and improves facial balance, providing a lean, feminine enhancement.
Neck Liposuction can be incorporated into a treatment plan for MTF facial feminization surgery as a defined jawline naturally provides a more feminine and youthful appearance. Neck liposuction can be carried out in combination with Facetite for patients looking for additional skin tightening. We also combine neck liposuction with submentalplasty, a surgical neck lift to help reshape an obtuse shaped neck.
Buccal fat removal can provide greater definition to the cheeks and subtly create a heart shaped face.
Excess fat in the lower cheeks is extracted through an incision intra-orally, removing approximately ½ – 1 tablespoon amount of fat. Your surgeon has to be cautious not to remove too much fat as taking too much fat can age your appearance when you are older.
Buccal fat removal is commonly performed as part of facial feminization surgery in younger patients to help highlight the cheekbones and reduce fullness in the lower cheeks.
Older patients making the transition later in life may benefit from facelift surgery as part of the facial feminization surgery. Rhinoplasty, also known as a nose job, is one of the most important and significant procedures to consider as part of your transition, as the nose is such a prominent facial feature.
If you decide to undergo more than one surgical procedure, rhinoplasty is often the first surgery performed, however it can be done in conjunction with brow reduction, brow lift, jaw reduction, chin implants, blepharoplasty, laryngeal shave as well as cheek implants. Many patients find the combination of a few surgeries to be the most impactful way to achieve the aesthetic results they are greatly seeking. A larger procedure known as forehead reshaping is also an option and a more significant surgery that can be performed for an even more dramatic result.

This procedure includes Brow reshaping, as a lot of patients for this procedure have heaviness to the brow, so we take the brow down and make it smooth. At the same time we often lift the brow to create a bit of an arch and to feminize the upper third of the face. Rhinoplasty is a common operation as well where we reduce the size of the nose and reduce the bulbosity of the tip which significantly feminizes the face.
Fat Grafting, Chin Reduction and Laryngeal Shaving which reduces the adam’s apple are also common operations we usually perform in facial feminization. Some patients undergo these procedures in combination, all at once, or a few at a time. We will work with these patients to achieve their end goal.
There are many surgical procedures that can help feminize the male face. Rhinoplasty and Tracheal shave are commonly sought out as they each are central features of the face and neck.
Additional surgeries often performed for facial feminization, include brow reduction, brow lift, hairline lowering, neck liposuction, buccal fat removal, lip lift and fat grafting. Non-surgical treatments such as filler injections to augment the cheek and lips can also enhance surgical results.

Common surgical risks include bleeding, swelling and patient satisfaction
Typically, we recommend our FFS patients take 2 weeks to recover from their combined surgical procedures. Bruising and acute swelling can last for 7-14 days. However, lingering swelling can take up to 9 months to one year to completely resolve depending on the FFS procedures performed. Sutures are typically removed 7-10 days following surgery.
